Your First Visit
We look forward to getting to know you and your family. When you schedule, ask us to send you a New Patient Information Packet. It includes forms about you and your health history, along with a map to help you find us.
At your first visit, you’ll usually meet with a nurse and a doctor. We’ll talk about your health history, listen to your concerns and answer your questions. We want you to feel comfortable and understand the next steps in your care.
If you need another appointment, we’ll help you schedule it and send a reminder as the date gets closer.
What to Bring
- Your insurance card, if you have one. If you’re uninsured, we can talk through payment options and available assistance.
- A photo ID, such as a driver’s license, state ID or passport. Don’t have one? Call ahead or ask us about other options.
- Health history information, including any medical records you have available.
- A list of your medicines and supplements, or the bottles themselves.
- Your questions and concerns. Writing them down can help you remember what you’d like to discuss.
