Community Health Worker
What is cHW?
A Community Health Worker is a public health worker who has an expanse knowledge of the community they serve. We build a long lasting relationship with patients to serve as a link/liaison between health and social services. We will facilitate access and make sure all of the patients’ unique needs are addressed. We also work as a wellness mentor to aid in Center for Family Medicine’s patients health goals and care plans.
RESPONSIBILITY TO PATIENTS
- Provide resources, outside help and educational information. (i.e. support groups, online apps, prescription assistance programs)
- Be an accountability partner on their health goals. (i.e. lose 5 pounds in 6 weeks)
- Consult with team; nurses, counselor, doctor, pharmacist, and schedule needed appointments. (i.e. high bps, medication management, other issues)
- Schedule referrals and outside appointments to facilitate and manage patients’ complete health.
- Reminder letters, calls and check-ins with patients to help aid continuity.
- Coordinate home visits to help manage patients’ chronic needs that lack transportation or mobility.
